In order to apply for a passport, it is mandatory to schedule an appointment through the Prenot@mi website. Please note that no fee is due to schedule an appointment. Appointments must always be confirmed by the applicant, using the online portal Prenot@mi, 3 to 10 days (by 12 pm Rome time) prior to the scheduled appointment date.
The appointment can be scheduled with either parent’s account, specifying in the notes that the appointment is for the minor child.
Passport requirements:
- to be an Italian citizen and, in case the minor was born abroad, having registered the birth certificate at the competent Italian municipality through the Consulate (see the Vital Records page);
- to be registered in AIRE.
Required documents to apply:
On the day of the appointment (booked through the Prenot@mi portal) it is necessary to attend in person and bring the following documentation:
- application form duly completed in its entirety (Application Form);
- the parent unable to attend the appointment in person at the Consulate must fill out and sign the consent form and attach a copy of his/her passport (page containing personal identification details and holder’s signature). If the absent parent is not an EU citizen, the signature on the consent form must be authenticated by a Notary Public;
- 2 passport pictures (recent, identical, passport size, white background, frontal, neutral expression, preferably no glasses);
- previous Italian passport, unless it is first passport, or loss/theft (in which case, you need to show another valid identification document);
- for minors holding only Italian citizenship: copy of entry Visa or United States Permanent Resident Card;
- for minors holding dual citizenship (Italian and U.S./other): copy of the other passport;
- copy of both parents’ passports (page containing personal identification details and holder’s signature);
- Until 12/31/25, money order of $135.60 addressed to the Consulate General of Italy in Miami (we do not accept money orders issued by Bank of America);
- an envelope (U.S. Postal Service Priority Mail or Express Mail) that needs to be pre-stamped and pre-addressed (indicating as recipient the address to which you want the passport to be sent) attaching the Release Form for the shipment of the passport.
FOR CHILDREN UNDER 12 YEARS OF AGE, IT IS POSSIBLE TO APPLY FOR A PASSPORT BY MAIL by submitting the following documentation to the Passport Office of this Consulate General:
- Passport Application Form for Minors (Application Form), fully completed in all its parts with signatures authenticated by a Notary Public (for residents outside the United States of America, the authentication of signatures must be apostilled);
- Two passport-size photos (recent, identical, passport format, white background, front-facing and centered, neutral expression, preferably without glasses);
- Authentication form for one of the two passport photos (Form), authenticated by a Notary Public (for residents outside the United States of America, the authentication of the photo must also be apostilled);
- Previous Italian passport, unless it is a first-time passport application or in case of loss/theft (in this case, another valid identification document must be presented);
- For minors who only hold Italian citizenship: a copy of the entry visa or United States Permanent Resident Card;
- For minors with dual citizenship (Italian and U.S./other nationality): a copy of the other passport;
- copy of both parents’ passports (page containing personal identification details and holder’s signature);
- Money order of $135.60 payable to the Consulate General of Italy in Miami (money orders issued by Bank of America are not accepted) — valid until 12/31/2025;
- an envelope (U.S. Postal Service Priority Mail or Express Mail) that needs to be pre-stamped and pre-addressed (indicating as recipient the address to which you want the passport to be sent) attaching the Release Form for the shipment of the passport.
