Requirements for passport issuance:
- be an Italian citizen;
- be registered with AIRE with an updated address in this Consular District, consisting of: the states of Florida, Alabama, Mississippi, Georgia, South Carolina, the Bahamas, Jamaica, the Cayman Islands, Puerto Rico, the Dutch islands of St. Maarten, St. Eustatius, and Saba, the British Virgin Islands, and the Turks and Caicos Islands;

- If the person applying for a passport was born outside Italy, it is necessary to ensure that their foreign birth certificate has been transcribed in Italy in the appropriate Civil Registry managed by the Municipality.
Documents required to apply for a passport:
On the day of the appointment (which can be booked via the Prenot@mi portal), you must appear in person with:
- for everyone: passport application form/substitute declaration, duly completed in all its parts (application form/substitute declaration);
- for everyone: 2 passport photos (recent, identical, passport size, white background, face front and centered, neutral expression, preferably without glasses);
- for everyone: money order made out to the Consulate General of Italy. The cost of the passport until December 31, 2025, is $135.60. Payment can only be made by money order (money orders issued by Bank of America are NOT accepted);
- for applicants with Italian citizenship only: copy of Resident Card or entry visa;
- for applicants with dual citizenship (Italian and US): copy of US passport and naturalization certificate (same documentation in case of acquisition of other citizenship);
- previous Italian passport, unless it is your first passport, or in case of loss/theft (in which case, show another valid identification document);
- for those born outside Italy: if your foreign birth certificate has not been sent to Italy for transcription by the Consulate General in Miami, provide a summary extract of your birth certificate (estratto per riassunto dell’atto di nascita). You can request this document to your local Municipality (Comune of AIRE registration) in Italy. It constitutes proof that your foreign birth certificate has been transcribed in Italy. If the municipality confirms that the foreign birth certificate has not been transcribed in Italy, please take immediate action to have it transcribed BEFORE making an appointment for the issuance of a passport;
- an envelope (U.S. Postal Service Priority Mail or Express Mail) that needs to be pre-stamped and pre-addressed (indicating as recipient the address to which you want the passport to be sent) attaching the Release Form for the shipment of the passport.
Passport applications may also be submitted by non-residents of this consular district only in cases of proven emergency occurring during a temporary stay in this consular district. In this case, the issuance of the passport is subject to authorization by the Police Headquarters or the Consular Office competent for the applicant’s place of residence.
