{"id":4952,"date":"2024-12-12T14:24:04","date_gmt":"2024-12-12T19:24:04","guid":{"rendered":"https:\/\/consmiami.esteri.it\/?page_id=4952"},"modified":"2026-03-30T14:46:19","modified_gmt":"2026-03-30T18:46:19","slug":"minor-passport-application-2","status":"publish","type":"page","link":"https:\/\/consmiami.esteri.it\/en\/servizi-consolari-e-visti\/servizi-per-il-cittadino-italiano\/passaporti-e-carte-didentita\/minor-passport-application-2\/","title":{"rendered":"Minor passport application"},"content":{"rendered":"<header class=\"entry-header\"><\/header>\n<div class=\"entry-content\">\n<p style=\"text-align: justify;\">In order to apply for a passport, it is mandatory to schedule an appointment through the\u00a0<a href=\"https:\/\/prenotami.esteri.it\/\">Prenot@mi<\/a>\u00a0website.\u00a0<strong>Please note that no fee is due to schedule an appointment<\/strong>.<\/p>\n<p>Appointments for minor children can be booked through the Prenot@mi portal using one of the parents&#8217; accounts and entering the child&#8217;s name in the notes field (specifying first name, last name, place, and date of birth).<\/p>\n<p>If you have <strong>more than one child<\/strong>, you will need to make a <strong>multiple appointment<\/strong>, registering the children as \u201caccompanying persons.\u201d<\/p>\n<p>Please note that for the renewal of passports for those <strong>under the age of 12<\/strong>, the procedure must be carried out <strong>by mail<\/strong> (<em>see below<\/em>).<\/p>\n<p style=\"text-align: justify;\"><strong>Passport requirements:<\/strong><\/p>\n<ul style=\"text-align: justify;\">\n<li>to be an Italian citizen and, in case the minor was born abroad, having registered the birth certificate at the competent Italian municipality through the Consulate (see the\u00a0<a href=\"https:\/\/consmiami.esteri.it\/en\/servizi-consolari-e-visti\/servizi-per-il-cittadino-italiano\/vital-statistics\/how-to-register-a-birth-2\/\">Vital Records\u00a0<\/a>page);<\/li>\n<li>to be registered in AIRE.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong>Required documents to apply:<\/strong><\/p>\n<p style=\"text-align: justify;\">On the day of the appointment\u00a0(booked through the\u00a0<a href=\"https:\/\/prenotami.esteri.it\/\">Prenot@mi<\/a>\u00a0portal) it is necessary to attend in person and bring the following documentation:<\/p>\n<ul style=\"text-align: justify;\">\n<li>application form duly completed in its entirety (<a href=\"https:\/\/consmiami.esteri.it\/wp-content\/uploads\/2023\/10\/dichiarazione-sostitutiva-1.pdf\">Application Form<\/a>);<\/li>\n<li>the parent unable to attend the appointment in person at the Consulate must fill out and sign the\u00a0<a href=\"https:\/\/consmiami.esteri.it\/wp-content\/uploads\/2023\/10\/modulo-atto-di-assenso-1-2.doc\">consent form<\/a>\u00a0and attach a copy of his\/her passport (page containing personal identification details and holder\u2019s signature). If the absent parent is not an EU citizen, the signature on the consent form must be authenticated by a Notary Public;<\/li>\n<li>2 passport pictures (recent, identical, passport size, white background, frontal, neutral expression, preferably no glasses);<\/li>\n<li>previous Italian passport, unless it is first passport, or loss\/theft (in which case, you need to show another valid identification document);<\/li>\n<li>for minors holding only Italian citizenship: copy of entry Visa or United States Permanent Resident Card;<\/li>\n<li>for minors holding dual citizenship (Italian and U.S.\/other): copy of the other passport;<\/li>\n<li>copy of both parents\u2019 passports (page containing personal identification details and holder\u2019s signature);<\/li>\n<li>money order of $136.30 (until 06\/30\/2026) addressed to the Consulate General of Italy in Miami <strong>(we do not accept money orders issued by Bank of America);<\/strong><\/li>\n<li><strong>an envelope (U.S. Postal Service Priority Mail or Express Mail)<\/strong>\u00a0that needs to be pre-stamped and pre-addressed (indicating as recipient the address to which you want the passport to be sent) attaching the\u00a0<a href=\"https:\/\/consmiami.esteri.it\/wp-content\/uploads\/2023\/10\/modulo-liberatoria.doc\"><strong>Release Form<\/strong><\/a>\u00a0for the shipment of the passport.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong>FOR CHILDREN <span style=\"text-decoration: underline;\">UNDER 12 YEARS OF AGE<\/span>,<\/strong>\u00a0<strong>IT IS POSSIBLE TO APPLY FOR A PASSPORT <span style=\"text-decoration: underline;\">BY MAIL<\/span><\/strong> by submitting the following documentation to the Passport Office of this Consulate General by mail (U.S. Postal Service Priority Mail or Express Mail):<\/p>\n<ul>\n<li style=\"text-align: justify;\"><strong>Passport Application Form for Minors<\/strong>\u00a0(<a href=\"https:\/\/consmiami.esteri.it\/wp-content\/uploads\/2023\/10\/dichiarazione-sostitutiva.pdf\">Application Form<\/a>), fully completed in all its parts with signatures authenticated by a Notary Public (for residents outside the United States of America, the authentication of signatures must be apostilled);<\/li>\n<li style=\"text-align: justify;\"><strong>Two passport-size photos<\/strong>\u00a0(recent, identical, passport format, white background, front-facing and centered, neutral expression, preferably without glasses);<\/li>\n<li style=\"text-align: justify;\"><strong>Authentication form for one of the two passport photos<\/strong>\u00a0(<a href=\"https:\/\/consmiami.esteri.it\/wp-content\/uploads\/2024\/12\/autentica-foto-inglese-italiano.docx\">Form<\/a>), authenticated by a Notary Public (for residents outside the United States of America, the authentication of the photo must also be apostilled);<\/li>\n<li style=\"text-align: justify;\"><strong>Previous Italian passport<\/strong>, unless it is a first-time passport application or in case of loss\/theft (in this case, another valid identification document must be presented);<\/li>\n<li style=\"text-align: justify;\"><strong>For minors who only hold Italian citizenship<\/strong>: a copy of the entry visa or United States Permanent Resident Card;<\/li>\n<li style=\"text-align: justify;\"><strong>For minors with dual citizenship (Italian and U.S.\/other nationality)<\/strong>: a copy of the other passport;<\/li>\n<li style=\"text-align: justify;\"><strong>copy of both parents\u2019 passports<\/strong>\u00a0(page containing personal identification details and holder\u2019s signature);<\/li>\n<li><strong>money order<\/strong> of $136.30 (until 06\/30\/2026) addressed to the Consulate General of Italy in Miami <strong>(we do not accept money orders issued by Bank of America);<\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>an envelope (U.S. Postal Service Priority Mail or Express Mail)<\/strong>\u00a0that needs to be pre-stamped and pre-addressed (indicating as recipient the address to which you want the passport to be sent) attaching the\u00a0<a href=\"https:\/\/consmiami.esteri.it\/wp-content\/uploads\/2023\/10\/modulo-liberatoria.doc\"><strong>Release Form<\/strong><\/a>\u00a0for the shipment of the passport.<\/li>\n<\/ul>\n<\/div>\n","protected":false},"excerpt":{"rendered":"In order to apply for a passport, it is mandatory to schedule an appointment through the\u00a0Prenot@mi\u00a0website.\u00a0Please note that no fee is due to schedule an appointment.
