• if the report card does not state that the student was promoted to the following grade, then a statement issued on school stationery, signed by the principal and attesting to the promotion to the following grade, must be attached to the report card;

Italian translation of the report card. The translation must be complete, accurate, and clearly typed. Translations of transcripts must show the grades exactly as they are on the originals;

• copy of passport;

• application form for Declaration of Value.

2. High School (only for US residents, not for Italian exchange students)

4. COLLEGE – UNIVERSITY (ASSOCIATE – BACHELOR – MASTER – Ph.D)

The person requesting the evaluation must send to the Student Office of this Consulate General a brief cover letter (including his/her phone number and/or email address) with his/her request, together with the following:

• original “official Transcript”, signed by the certifying school official, with attached to it the “Apostille” of the Secretary of State legalizing the signature of the above-mentioned school official* (in order to obtain an Apostille on a document, the school official’s signature must be authenticated by a notary public);

Original Transcripts and Diploma of the previous title (i.e.: if requesting a Declaration of Value for Bachelor or Associate degree, the previous title is High School diploma). If the qualification was obtained in a State outside of the consular district of Miami, it is necessary to obtain the declaration of value issued by the competent consular office.

• in the case of a Ph.D., besides the above, the requesting party must also submit University documentation explaining admission requirements to the school’s Ph.D. program, minimum and maximum duration of the program, graduation requirements (discussion of a thesis, publication of a thesis, etc.);

• copy of passport;

• application form for Declaration of Value.

* In order to obtain an “Apostille” on a document, the school official’s signature must first be authenticated by a notary public. Please note that neither a parent, nor an exchange program coordinator, nor the student him/herself may attest that the document is an original or a copy and then have his/her signature notarized, and then legalized with an Apostille. This would be a waste of time and money because THE AUTHENTICATED SIGNATURE MUST BE THAT OF THE SCHOOL OFFICIAL WHO SIGNED THE DOCUMENT.

GENERAL INFORMATION

Processing time is approximately four to seven weeks, if the documentation is complete. All documents will be returned by regular mail. A prepaid addressed envelope must be enclosed in your application. This office does not accept checks, nor overseas C.O.D. shipments.

The consulate will mail the documents only to the requesting party, in the U.S. or in Italy, or to the host parents, but not to schools or exchange organizations.

Do not ask the school, nor the Secretary of State to send the documents directly to the Consulate.

If the consular evaluation is requested for work purposes and not for continuing education, consular fees apply. Please contact the consulate to find out what the current fees are.
